Dell recommends that you upgrade the firmware for the internal H710 RAID controller. It will require 5-10 minutes to install the update and require a 10 minute reboot.
Download the PERC RAID Controller firmware for your specific system. Look for the Red Hat Linux Update Package (.BIN). For Example:

Download the Red Hat Linux Update Package (.BIN) file to /home on the sensor.
To update the firmware do the following:
- Login as root.
- Shutdown Security Analytics using: scotus stop
- To install the firmware:
- chmod 755 /home/SAS-RAID_Firmware_XXXXX_LN_VERSION_AXX.BIN
- Update the firmware:
- /home/SAS-RAID_Firmware_XXXXX_LN_VERSION_AXX.BIN
This will take a few minutes, There will be verification that the firmware is the correct firmware for the hardware and lots of license details. Once it is done, it will reboot the system.
SA should come online after the reboot.

This behavior has been corrected in M5100 Series SAS/SATA
Controller firmware version 23.33.0-0033, and M5200/M1200 Series

and operating system on IBM Support’s Fix Central web page, at the

Workaround
No hardware should be replaced. The ServeRAID controller should
be updated to the latest firmware level.
Additional information
When the ServeRAID SAS/SATA controllers are running in
Integrated MegaRAID (iMR) mode, with no Transportable Memory Module
(TMM), a certain amount of system memory is allocated to controller
so that out of band commands can be performed.
With the previous firmware, there was no check in place to
verify buffer overruns did not occur within this available memory
pool.

Проблемы с массивом на LSI9280

Нужен совет
Есть сервер, fujitsu primergy rx200 s7 wi2012r2std, установлен контроллер LSI 9280-8e, подключена полка на 24 диска, собран массив raid60
С некоторых пор начались проблемы — диск пропадает из системы. Вслед за диском пропадает и контроллер

После перезагрузки работа восстанавливается и некоторое время все работает нормально
Попробовал заменить контроллер на 9286CV-8e, ничего не изменилось
Вдобавок после одной из перезагрузок контроллер потерял диск. Диск я заменил, запустился ребилд, но идет он как-то очень вяло
Собственно, вопрос — чтоможно предпринять? Заменить сразу два диска с ошибками чтения мне не кажется хорошей идеей

Как я понимаю, контроллер «левый»? С дисками, надо проверить те, что вызывают подозрения. Если есть какие-то косяки — менять. Были случаи, когда один диск «гадил» всему массиву.

По проверке дисков: что будет, если я проблемные диски просто выдерну и заменю? По факту проблемные диски в разных спанах, проблем быть не должно. В теории.

По одиночке меняйте, дожидайтесь полного ребилда. Можно еще прогнать верификацию рейда. А то, что в разных спанах, не играет особой роли. Я повторюсь, у нас была пара случаев, когда один диск гадил всем. Правда на Адаптеке, но не суть. Чем-то «забивал» контроллер, у того постоянно вываливались диски. На форуме редко, но тоже проскакивали случаи.

Но гадость ситуации в том, что таких проблем вообще не должно быть (пропадание контролера). Так что поаккуратнее.
Спонтанный отвал исправных дисков чаще всего бывает из-за проблем совместимости или кабелей/бэкплейнов…
В общем, подергать провода, прошить контроллер, если не свежий… после окончания ребилда конечно.

Эта полка с дисками на таком же контроллере долго работала без нареканий

Тут даже кейс не откроешь, и никто не будет гарантировать работоспособность. Хотя конечно работать то должно бы. Контроллер стандартный.

Мда, это печальненько, это под нагрузкой из под ОСи? Как вариант, все отключить, контроллер воткнуть в другую машину и попробовать на нем под биосом, без нагрузки.

А вот в биос контроллера мне зайти не удалось. Странно. Такое впечатление, что при загрузке сервер его вообще не видит
Перезагрузился, остановил все процессы — 13 часов

Ребилд прошел. Заменил оба проблемных диска, массиву сразу полегчало — на ребилд хочет 16 часов и это под рабочей нагрузкой.
Ошибки типа «Corrected medium error during recovery on disk» еще есть, но их немного

How do I Resolve the «Controller ID: 0 Fatal firmware…» Error?
Environment
Intel® RAID Controller RS2BL040 / 80
Server board with the latest BIOS/firmware
Troubleshooting hints on how to approach the «Fatal Firmware» errors.
When the Controller ID: 0 Fatal firmware… error occurs, the system reboots.
The fatal firmware error initially points to the controller having failed. However, consider the following steps first:
- See that the Board-Card combination is a compatible one.
- Contact Support with the RAID logs and System Information Retrieval Utility (Sysinfo) logs if the situation persists.

If problem still occurs then disable consistancy checks in MSM,
Disable any extra motherbd sata controllers (marvel, jmicron, gigabyte, asmedia…) and if you still get the problem then set your intel/amd ports to ide mode or disable them too. If you still have problems you probably have to send the card in for repair.

Note:
installing the kernel source of Centos 6.3, copying the 05.40 megaraid_sas version for RHEL 6.3, recompiling the kernel and using this version of megaraid_sas, the machine was very stable, occurring only 1 freeze using bonnie++. The situation was more stable I got so far in this environment.
